He lost about fifteen pounds in two to three weeks, and it was just insane. I had a terrible attitude, which was totally odd for me because Im actually a nice person. Especially being on the road around people all the time, you have to keep that positive energy going. And it was hard. Whats awesome about the OmniPod, Nick says, is that it works as a FreeStyle meter, so I just put the FreeStyle test strip in it. He tries to check about nine to ten times a day, and his blood sugar is usually 150 to 175 mg/dl. He acknowledges this is high, but because he is so fit and active and has a tendency to go low, this is what he has been advised should be his normal range. His friends are very supportive.   • Diabetes, the treatable epidemic that is failing most patients.

    The topic herein relates only to Type II diabetes mellitus (sugar diabetes) or adult onset diabetes. Diabetes is difficult to manage; it can be unfulfilling. So far it has been one of my major clinical failures. Diabetes expresses itself through complex factors that include genetic predisposition, age, obesity and activity. Most patients with chronic illnesses are often depressed. The medical model, as commercially structured, does not lend itself to address issues of the heart or those that impact our patients with anxiety and/or depression that may diminish compliance. The modern medical model focuses on unaffordable major ticket items that most often yield little in terms of value (life over cost). We should not see the American Hospital Association (AHA), The American Medical Association (AMA), big Pharma (drug industry) and insurance companies as useful in my quest for a quality universal health care plan with choice. According to the New England Journal of Medicine (2008), 40 patients with carotid stenosis would have to be expensively treated to prevent just one major stroke over a 5-year period. Thus, to help one patient, we must treat 39 that derive no benefit but costs millions. Political leaders want to protect the health care industry on behalf of stockholders and investors at the expense of human suffering and health care access for all. The uninsured does not have a group that lobbies on its behalf. Where are religious leaders in regard to this issue? Returning to the topic of diabetes, according to The Medical Letter (2008), The ACCORD Trial, intensive glucose lowering in Type 2 diabetes, including frequent use of thiazolidinediones, mostly rosiglitazone (Avandia®) and insulin, with an HbA1c target of 6.0% did not obtain a major reduction in major cardiovascular events over a period of 3.5 years. The ACCORD trial was stopped early because of an unexpected increase in all-cause mortality in intensively treated patients. By accessing the FDA, consumers can learn the actual risks associated with drugs like Avandia®, which include upper respiratory tract infections, injury, edema (fluid retention), weight gain, changes in serum lipids and risk of fractures in women. The commercial medical model and investors benefit if we simply stay fat and inactive though many of the expensive drugs they market to us are simply dangerous and often useless. Diabetes is the end result of a life long set of poor eating habits and poor choices. Diabetes cannot be addressed unless we also address the cultural issues associated with eating. Our body can derive energy from fat, protein and sugars (carbohydrates). Excess carbohydrates are converted into fat. Carbohydrates are important to the bone marrow, red blood cells, white blood cells, the renal medulla, eye tissues and peripheral nerves. The brains preferred energy source is glucose. In the absence of glycogen, the bodys glucose level is maintained by complex mechanisms that include protein breakdown and triacylglycerol from fat. Sick children readily use up their glycogen stores and develop ketosis from fat metabolism with associated nausea, vomiting and abdominal pain. By simply providing such children with oral sugar, the ketosis is reversed and the symptoms resolve. The Mediterranean diet with low carbohydrates may be an effective alternative to low-fat diets with more favorable effects on lipids and glycemic control (NEJM-2008). By reducing the daily sugar intake to 150 grams per day (600 Calories), reducing total caloric intake and by increasing daily activity, many of my motivated patients have achieved early glycemic control, usually without drugs. When I prescribe drugs, I limit my repertoire to two second generations sulfonylureas, GLIPIZIDE or GLYBURIDE and METFORMIN (an antihyperglycemic, not hypoglycemic). Metformin reduces glucose levels primarily by decreasing hepatic glucose production and by increasing insulin action in muscle and fat. If my patients fail to respond to psychotherapy, diet and the drugs aforementioned, I simply refer them to other providers that can simply manage the condition as a progressive disease with devastating consequences that often includes blindness and renal failure. Many of the patients I refer out to other providers later wish to return to me and comply to resolve their condition in a more sound and safe manner. Doctors must not prescribe expensive combination drugs like Glucovance® which consists of GLYBURIDE and METFORMIN. I may simply start a patient individually on GLIPIZIDE or GLYBURIDE and add METFORMIN (generic) as appropriate.
